STIPULATION FOR RELEASE OF PLAINTIFFS RECORDS AND INFORMATION | |
COMES NOW the parties to this action and hereby stipulate to provide information by and between each other in a contemporaneous fashion condition upon the stipulations set forth below:
- Defendant agrees to produce contemporaneously any information whether school records, medical records, tax records, employment records or any information procured from use of executed releases.
- Parties stipulate that any information obtained by use of releases shall be used solely for purposes of this litigation.
- Any individual or Insurer obtaining information in disclosures or from use of a release agrees to maintain the confidentiality of this information. Said information shall not be discussed or disseminated except in connection with proof and/or defense of this litigation.
